Backpacks for Fieldcrest '26 - Supply Request Form

If your child is in need of school supplies for the 2026-27 school year, we would love to help you out. Please complete this form ASAP, but no later than JULY 17th. *Please complete one form per child.

With your registration, you and your student are invited to join us on Wednesday, July 22nd for "Pack the Bag" open house. Come anytime from 5-7pm. Your student will get to pick their backpack and fill their bag with the needed supplies.
Location: Redeemer Community Church . 568 N Chestnut St . Minonk (next to the Sweet Shop)
